Permanent Supportive Housing

The following programs provide long-term financial assistance and supportive services for individuals and families who are experiencing long-term homelessness with one or more disabilities, such as mental health, substance abuse, or a physical disability. To be eligible, individuals must be literally homeless (living in a shelter or on the streets) and have a disability. Referrals are accepted through the community’s coordinated entry system.

Heartland Housing Beginnings
To be eligible, individuals must be ages 18-24 who are literally homeless, have a disability, and are former wards of the state. They must live in Douglas and Sarpy counties in Nebraska.

Heartland Housing Solutions
To be eligible, individuals or families must be literally homeless, have a disability, and live in Pottawattamie County in Iowa or Douglas and Sarpy counties in Nebraska.

Samaritan Housing
To be eligible, individuals must be literally homeless, have a disability, and live in Pottawattamie County in Iowa or Douglas and Sarpy counties in Nebraska.

Heartland Homes/Transitions
Provides facility-based, long-term housing to individuals and families in Council Bluffs, Iowa who are literally homeless and have a disability.
